PDF design formatting issue

I prefer to use the old pdf design option, however, every time I try to modify the pdf design for our form: "2021 Staff Application Form (Returning)", it jumbles all the fields on the pages. Instead, I would prefer to have the fields formatted in the same was it was previously, which is straight down the left side of the page, question below question. Are you able to help me with this?

If that's a case, you can create a new PDF document with correct layout by following these steps below:

As for the PDF editor, as I can check, you already switched to the old version.

Now that I have created a new PDF design template, I need to connect it to the form so that it attaches as a PDF. I cannot seem to find how to do this... Please advise. I have gone though the settings but cannot seen to locate this option. Thank you!
