Hello,
Can we send in a scanned copy of our current patient information and medical history forms and have a you create online forms from these for our website. Thanks.

To verify, are you referring to sending a copy of the PDF submissions to patients? My apologies for the confusion. Can you provide us with additional details on your requirements?
However, you can attach PDF submissions to autoresponder emails to provide copies to your clients as a PDF attachment. For HIPAA enabled forms, you would need to create a strong password for the PDF documents:
For HIPAA accounts, PDF Attachments are allowed only if you provide a strong password. Your password must contain a minimum of 8 characters, and it must be a mix of upper case, lower case, numbers, and special characters. Please write the password you want to use.
7- Click on “Save” to save changes.

Hi Kris,
Sorry for the misunderstanding. I was asking if we could you send you a PDF of the current new patient forms we have patients fill out by hand and have your team create a form fillable electronic form on our office website with the same information we have on the paper forms. Does that make sense? Thanks.

If the form is in PDF, you can convert your form into a PDF Fillable Form by importing the form. Please refer to this guide: how-can-i-import-my-own-pdf-with-smart-pdf-forms
Then you can embed the PDF Fillable Form to your website: embedding-a-form-to-a-web-page
We usually advise users to use the iFrame embed codes to prevent conflicts on third party website script codes because the codes of the form are called inside an iframe: How-to-Get-the-iFrame-Code
