Account Suspended

Hi, I'm Hamza Azam i'm Provide Jotform Building Service on fiverr and upwork i have done many orders and get many Clinets to jot form but recently account of my Clinet is suspended i don't know whats happen please help me out here

I have checked the account dancefloorhireco and it seems the form 203001306715035 is collecting credit card information which violates our terms of use.

If you want to receive payments in your form, you would have to use the payment integration.

To lift the suspension in your account, we would have to delete the guilty fields in your form.
