I just upgraded to the silver plan and now I can't find our patient form template. WE had the template housed on our website and when I select the link now it gives me a Whoops! message. I need to make sure that when patients go to our website they can access the form. I need to restore this asap. Thanks.

Thank you for contacting Support.
Upon checking, you have recently changed your username.
If you have any forms with custom URL links, you will need to re-create the URL with the new username as the old link will not be active. However, direct links will work fine.
