HIPAA: form is getting a "Temporarily Back-end Unavailable" when it loads

    This error message is showing up where a form is supposed to be. I hit refresh and it does not load the form. My client needs this form to be working ASAP.

    I have tried on Safari and the form is loading but the form will not load on Chrome. The error message continues to show instead of the form.

    Once we have further information, you will be notified at this support ticket.

    I have checked your form again on Chrome and FireFox and it seems to be loading already.

    Ok. I just cleared my cache and the form is loading now.
