Hey everyone,
I'd like to know whether it's possible to CREATE a form based on another form's input.
Example:
I want to give my clients a form to fill up which has fields like...
- Name
- Email
- Institution EmailBased on that input JotForm will automatically create a form which they can shared to their clients so that they can book appointments with them without me having to manually create it.
All I have to do is, send them the information request form, once they submit it they will have their own form.

It is not possible to create a form based on inputs. You would have to manually create a form in the form builder.
It is possible to populate a form field data using URL parameters. Guide:https://www.jotform.com/help/71-prepopulating-fields-to-your-jotform-via-url-parameters
