    Hey everyone,

I'd like to know whether it's possible to CREATE a form based on another form's input.

    Example:
I want to give my clients a form to fill up which has fields like...
    - Name
    - Email
    - Institution Email

Based on that input JotForm will automatically create a form which they can shared to their clients so that they can book appointments with them without me having to manually create it.

All I have to do is, send them the information request form, once they submit it they will have their own form.

    Hope this is clear.

    Thanks

It is not possible to create a form based on inputs. You would have to manually create a form in the form builder.

It is possible to populate a form field data using URL parameters. Guide:https://www.jotform.com/help/71-prepopulating-fields-to-your-jotform-via-url-parameters
