I think Cole_solarisaveda meant to ask is if we can make a password for the form that we created so the user has to enter a given username and password

( this is also the question I wanted to ask)

You can password protect a form as explained here: https://www.jotform.com/help/579-how-to-password-protect-a-form

Do review and let us know if this helps.

Yes, the guide shared above shows how to set a password for a form. However, it cannot be deleted. It can only be modified/reset by the form owner.

Apart from that, you cannot use username and password fields on the form, since then your account would get suspended due to phishing.

So what you can do is add a Short text entry field on the form and title it as Access code. Now using conditions show the rest of the form, only if the entered password is correct.

Here is a demo form: https://form.jotform.com/203032986819058

You can use the access code as "test123". Once you enter that in the Access code field, the rest of the form will show up.

Would this work out for you?
