    How can I ask a question on my form where ther are 3 options and any or all of them can be checked? So far it only allows one to be checked.

    Hello fistirling,

    You have added radio button question in your form. Only one item/option can be select from a list/set of radio button options.

    You should instead use check box questions to list your options. That will allow you to select multiple options.
