Why was my account suspended?

    Hello,


    My account got suspended and would like it reactivated. I work for a nationwide reputable credit repair company with thousands of clients. I will be collecting login information from them as well as the last 4 of their social security number. I will have them sign that they agree to have us use that info. Please let me know if you have any questions.

    Hi,

    Your account was suspended by our anti-phishing system due to collecting passwords and SSN information.

    It is not allowed to collect log-in credentials through our forms as it is against our terms.

    Also to collect SSNs you must add the following:

    1- A terms and conditions widget. https://www.jotform.com/widgets/terms-and-condition

    2- A signature field.

    We can only re-activate your account if you agree to us removing the fields mentioned above from your form and make sure to add the mentioned widgets once your account is re-activated.

    Go ahead and delete those fields. I'll add the widgets or not do the form at all.

    Why is my account suspended again. You deleted the fields and I completely deleted form. I was about to add a form requesting SOCIAL SECURITY info but as I was going to add Terms and conditions widget and a signature like you requested. What is the problem now?

    We would like to apologize for any inconvenience. I have checked the forms in your account further and found that there is a form in your account's Trash Folder that has the following fields, which we do not allow based on our terms.

    Gathering these types of data violates our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

    We’ve removed the offending fields on the form and lifted the suspension on your account. Please refrain from making new forms that are against our terms to avoid this kind of inconvenience.

    Thank you. So just to be clear I can collect login info if I ask for an
    access code rather then a “password”?

    I'm sorry for the confusion. But asking for login credentials within our forms is against our terms. You may use "Access Code" if, for example, you have a form that requires a correct access code to view the rest of the fields or the entire form.
