Add Signature and terms and conditions widgets to Form.

Hello,

We currently use JotForm on our pro website as an account application.

https://professional.chella.com/pages/wholesale-application

At the bottom of this application, there is a checkbox and link to our terms and conditions.

We would like to know how to take steps to make the terms and conditions a document that will require an electronic signature by the applicant.
