Great Lakes Heavy Haul - Covid Screening

When this form is downloaded from the email notification I receive the header is incorrect. It still displays the form header I duplicated it from. It should read "Great Lakes Heavy Haul - Covid Screening"

How do I fix this?

Hi,

Since you duplicated the old form, the old form's PDF template will be duplicated.

Please go into your new form's PDF editor and delete the PDF document for the old form.
