i didn't receive any submitions sense i use the form
i made the mail test from jotform.com and received mail successfully but my problem is that i didn't receive submitions from the site i host the form on
i made a static html and hosted it into my host server then i copied the embed code of my form and pasted it into this statitic html , then when i tried it online it didn't give any response
further >> i,m using this static html on facebook apps third party

Upon taking a closer look of your form's email alert, I found that you have not selected the "Sender E-mail". That seems to be the reason of you not receiving your submission emails.
You should select/add the "Sender E-mail" in your notification as this is required to be added for your notification alert to work.
You can select "firstname.lastname@example.org", "email@example.com" or you can also setup a custom "Sender E-mail" option. This guide should help you:
Please Note: Your test email went through just because the "Test Email" directly sends the email to your account email address and doesn't check for sender details. It comes into effect only when live submission is done.

I have the same problem with my form. it is hosted at http://www.bistroindia.ca/reservations.html
can you please check back and let me know what the problem is. test emails are going through to my gmail inbox but the live website submissions are not going through.
