Fillable PDF form within a form

I added a fillable form, It does allow the person to fill in the blanks however it does not save them to the submission and when submitted the blanks are empty. How do I fix this?

The PDF embedder widget does not work the way you are trying to use it. The PDF embedder does allow the PDF to be displayed within the form but it will not collect the data like this.

In order to receive data in the uploaded form, you will need to upload that PDF form separately as explained here:

How-Can-I-Import-My-Own-PDF-With-Smart-PDF-Forms

This will create a new form link for this uploaded/imported PDF form. You can get this form /URL and add that in the Thank you page of your Employment Application form as an external URL: redirecting-users-to-a-different-page

When they fill one form they will get redirected to a second form.
