Doctors stern advice.

    The three bottles arrived three days ago and l had to speak to my doctor and he told me that under no circumstances was l to have anything to do with this type of treatment and he really told me off for ordering it so l'm sorry to say that l have to return the three bottles back to you, they haven't been touched or opened so l will send them back as they came.

    But what l need is the address that l need to send them to, so can you send me your address that l can print off and send them back to you, so very sorry as l really thought that this would be the end of my problems.

    Hello Tony - Apologies, but I'm not quite sure about the concern. I assume that you have made an order to a form created with Jotform. Is that correct?

    If so, please note that we only host the forms created by form owners on our website, so if you have concerns with the services they offer, please contact them directly for further assistance. Usually they are the ones who sent you the link to that form or the owner of the website where the form is embedded.
