I'm trying to upgrade our free account to the Professional level. We are a higher education institution and need to pay by check or money order, but when I select that option on your form, the drop down box for payment type on the screen where you fill in the contact info only offers the option to pay by credit card. Please advise.Page URL:

Also, please make sure to provide your Jotform Username in the section that says "Jotform Login Name" and use the same email address as the one you've used to your Jotform Account. Having these information correct, would prevent future problems with your billing.
