We are encountering an increasing number of instances where people are failing (incomplete submission) to use pay using PayPal via one of our forms. What can I do to diagnose whether the problem is our form, our website, or PayPal?

I've check your form and base on your recent submissions, I think is no problem with the form. None either with your website and Paypal. The only possible cause of this is that your visitors refused to send their payments after being redirected to Paypal's Payment Page. Maybe, your visitors just fill out the form and never check your notes or the whole contexts of your webpage that they will be redirected to the payment page and will be asked to pay after clicking submit button.
How about an extra note just before your form's submit button? That should further tell them about what will happen next.
