I received a phishing email that uses a jotform link in hope of getting my credit card information. The main text of the message is as follows (note that I have made a purchase from MadPax LLC, and I've informed them that their customer database has been compromised - I also have a text file of the source):

MadPax & Trade Over Bill Refund
Dear Valued Shopper
We noticed there was an over-bill from our billing system to your credit card the last time you shopped with us at www.madpax.com As we earlier promised in our previous mail to refund your credit card with its
over-billed amount of $430.65.
All affected shoppers will receive this message to enable us verify their credit card for refund and stop further billings to your account.
Please follow the link below to verify your credit card information to enable us effect your refund to your credit card as soon as possible.

We apologize for any inconvenience this might have caused you, and we apologize for the delay.
