I created the form and everything sent properly. Now only some emails actually make it to their destination. I've removed every "condition" and rebuilt each one. I have 2 conditions set up for email notifications. Sometimes it goes to the "Agent" and sometimes it goes to the "Producer". Sometimes it goes to both, and sometimes neither. It is not a specific person that is failing to get them, and I've checked all of the email addresses multiple times. I have it forwarded to a google spreadsheet and every entry is being sent to it successfully, but some of the email notifications are not making it out.
I just went back in and flip-flopped emails for the producer and agent and it appears it won't send anything out to the "Agent" name.

May I suggest you also add a default Notification E-mail to your form.
This will allow you to properly monitor which e-notification with Condition is not delivered during form submissions. Because there will be 2 email notifcation everytime (1 for you, and another to agent/produce).
In the Agent Name field, try not to use comma(,) sign in the option selections
because this comma sign will sometimes cause issue in the condition logic codes.
Remove or replace the comma sign with space or dash(-) instead. When you do this, make sure to update also the condition email in the condition wizard.

copy as the administrator. Also, I've been testing both fields with the
TEST -Tom name so the commas shouldn't effect that name, but I'll change
them and see if it helps.

As long as you are aware and understand the logic of your e-mail condition, I think there is no need to create a separate email notification.
Currently, the email message that is given to both agent and producer is the same. So, if you will just create another notification email with the same content/message -- I think that will be redundant.
The logic condition email should be able to execute this enough.
