how to create USER ID by adding the first 4 characters of firstname + last 4 digits of IC number or Phone Number

Please note that what you want cannot be done using the Unique ID widget as shown on this guide.

However, you can add a Substring widget to get the first four characters of the name, then another Substring widget for the last 4 digits of your IC Number.
