Why did my form change?

    My form used to be 3 columns wide.  I've received a call that the columns no longer align correctly and when I try to edit it, my first column doesn't even show.  What happened?

    The moving and stroage checklist is the form having the problem on the rates and inquiry tab.  The room categroies have shifted and no longer align with the items in those particuar rooms and the form itself doesn't let me see those categories when I go i n to edit it.  HELP!

    We do not change our users' forms. Please check your account history using the account settings page:

    https://www.jotform.com/myaccount/history

    As far as I can see, the first column is actually depends on your web page code. It is not the part of the form.

    It is probably that you have changed the form or your web page layout, so the columns do not match now.

    Can you please check your web page layout with the web building application that you use for this website?
