    Hi,

You can use custom domain with JotForm, for example: www.yourdomain.com/yourform, it will show your form the same way as following the standar JotForm links like http://form.jotformpro.com/form/32487050540954 but with your own URL, in order to achieve this please follow this guide:

    http://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform

On the other hand, if what you need is to embed your form inside on your already created web page, you can use for example the iframe code generated by the form builder.

Please check this docs about adding a form to a web page:

    http://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page

Also check this other guides about differents ways of sharing your JotForm forms:

    http://www.jotform.com/help/chapter-3-Share+Forms

    hope it helps

please lte us know if you need further assistance

    thanks
