    if i use the embed source code for joomla; such as this:

    Hi,

    Please use the source code when embedding your form, to use the method in that user guide:

    Insert this code right after the </form> tag:

    <script>
    document.getElementById('input_5').value=location.href;
    </script>

    Alternatively, you may follow the instructions in this thread for an easier way to tell where a submission came from.
