Question 1) Can we integrate a payment feature with your form?
If a regional manager wants to complete a form for 7 different facilities - with each facility being a separate entry, can we do that? SEE Highlighted example below:
Parent Company ______________________
Facility Name ___________________________
Contact Phone #______________________
Contact Email _________________________
Non profit? Y/N
Type of equipment accepted ( nine option drop down menu - more than 1 option can be selected)
Type of services provided ( Multiple selection drop down menu - more than 1 option can be selected)
DO YOU HAVE ANOTHER FACILITY YOU WOULD LIKE TO ENTER AT THIS TIME?
IF YES - form would go back and ask all of the same questions for the next facility (This would repeat until they answered NO more facilities.)
IF NO - form would take them to a payment page where we could integrate payment feature.
