Do you have a feature that allows someone to complete multiple forms for multiple facilities in a single transaction?Asked by patty osterberg on September 19, 2013 at 12:36 PM
Question 1) Can we integrate a payment feature with your form?
If a regional manager wants to complete a form for 7 different facilities - with each facility being a separate entry, can we do that? SEE Highlighted example below:
Parent Company ______________________
Facility Name ___________________________
Contact Phone #______________________
Contact Email _________________________
Non profit? Y/N
Type of equipment accepted ( nine option drop down menu - more than 1 option can be selected)
Type of services provided ( Multiple selection drop down menu - more than 1 option can be selected)
DO YOU HAVE ANOTHER FACILITY YOU WOULD LIKE TO ENTER AT THIS TIME?
IF YES - form would go back and ask all of the same questions for the next facility (This would repeat until they answered NO more facilities.)
IF NO - form would take them to a payment page where we could integrate payment feature.
Thank you for contacting us with regards on this matter. In order to achieve that in your form, there are few things you need in your form.
