Forms quit working. We attached a picture of what happens after submit. The field is filled out that it warns against.
We tried to get into our account to find a solution and we are unable to see your website from any browser.
Will you please let us know if there is a problem you are working on?

The Seller Name Field ID from your form embed codes aren't the same as the one in your form direct URL. This sometimes happens when you modify a form and then forget to re-embed it using the same method. Please take note that the "Form Source Codes" won't automatically update if you will modify the form in our form builder. Using this method, you will have to re-embed the form everytime you'll do changes.
In this case, I would recommend re-embedding your form using our iFrame Method: How to Get Form iFrame Codes. This should fix the issue.

As for the problem accessing our website, please try to clear your forms cache and cookies: How to Clear Your Browser's Cache. If it has the same output after doing this solution, let us know by creating a new thread so we can check further.
