why is my form disabled ?

    Our automatic phishing detection system has suspended your JotForm account. Your forms are currently disabled.
    If you would like us to review this decision please reply to this email and provide these information:
    Describe the purpose of your forms. If you have asked for username or password in one of your forms, please describe the reason. Provide the URL of your web site where you use these forms. We will try to resolve this issue as quickly as possible.

    > The purpose of my form is to register a patient for my website www.drsumit.co.in where we provide telemedicine and online consultations. Patients are asked to enter their preferred username and password which will be given to them so that they can log in to the patient portal. 
    Please note we are asking for username and password for registration purpose only. Patients once registered will get a email from our side once we create their patient account and then they can log in using their userid and password. sending random userid and password often creates lots of issues patients usually tend to forget them and we again end up resetting them.
    Even if our welcome email is caught in spam box patients already know their username and password because they chose it themselves. I am a doctor by profession and have nothing to do with phishing

    Sorry for the inconvenience that may have caused. Upon checking your account, I found out that our phishing detection system suspended your account due to possible of your forms in phishing activities. Form titled "Forgot Password" causing the issue as it's asking for password and it's against our terms of use. I will now re-activate your account and make sure to remove/delete the said form to avoid this from happening again.

    hi jotform support 

    the form YOU REFER TO  only resets passwords

    for members who have forgotten their password.

    Am we wrong in doing that ????


    WE ARE NOT ASKING for others passwords

    We totally understand. However, Getting passwords and usernames is considered phishing and this is strictly prohibited by our terms of use.


    The problem with your account is  that it was automatically suspended by our phishing detection system because  you have this form which was found guilty of a phishing activity.
