JotForm Team______________________________________________________________________________________________________> The purpose of my form is to register a patient for my website www.drsumit.co.in where we provide telemedicine and online consultations. Patients are asked to enter their preferred username and password which will be given to them so that they can log in to the patient portal.Please note we are asking for username and password for registration purpose only. Patients once registered will get a email from our side once we create their patient account and then they can log in using their userid and password. sending random userid and password often creates lots of issues patients usually tend to forget them and we again end up resetting them.Even if our welcome email is caught in spam box patients already know their username and password because they chose it themselves. I am a doctor by profession and have nothing to do with phishing

the form YOU REFER TO only resets passwords
for members who have forgotten their password.
Am we wrong in doing that ????
