    Hi there!

    Your form is having conflict with existing scripts, re-embedding the form using iframe should fixed it.

    How to get your iFrame Embed Code

    Let us know if the problem still persist after that. Thanks!

    I can see that you manually encoded the script that starts from here  <form id="32658510314146" and ends here data-info="user_location"> that conflicts your script. I'm not sure how your scripts works but I have tried to replace the said script with an iFrame code and works perfectly fine.

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="//" frameborder="0" style="width:100%; height:1692px; border:none;" scrolling="no"></iframe>

    <script type="text/javascript">window.handleIFrameMessage = function(e) {var args =":");var iframe = document.getElementById("JotFormIFrame");if (!iframe)return;switch (args[0]) {case "scrollIntoView":iframe.scrollIntoView();break;case "setHeight" = args[1] + "px";break;}};if (window.addEventListener) {window.addEventListener("message", handleIFrameMessage, false);} else if (window.attachEvent) {window.attachEvent("onmessage", handleIFrameMessage);}</script>

    Form should looks like this:
