how can i add titles into my forms?

when i submit the forms, the tiles goes auto response with same text and same pictures,

is there anyway when guest are posting the form, the title are included so when it is post, the title will be what the posters are? help

Hi Reiahiz145145,

You can change or add a title by going to Settings > Form Settings > Scroll down > Page Title.

You can also add a Header by going to this at the top part.
