What did I do wrong?

I reviewed and tested your form but I was unable to reproduce the issue that you having. Can you let us know the browser that you are using when you get the error? Also would it be possible for you to send us a screen shot of the error that you received?

The form in question currently has Loose Check enabled. As such, it is allowing only one entry per IP. You'll need to change the setting to No Check. Please see this guide for details.

Sorry, I meant it's your browser cache that's preventing multiple submissions. Strict Check is the setting which checks the user's IP. Anyway, that's all explained in the guide.
