Hi and thanks for a speedy response. I've attached a screenshot of what I'm trying to achieve.
I’m trying to get the values from columns L, M and N to populate the single choice radio buttons within the “Replacement Cost” field. I cannot see how I should connect the field headers from the spreadsheet to the radio buttons on my form.

Hello,
Please note that the Spreadsheet to Form widget cannot assign a value to the options in the Radio button field. It can only auto select that option in the field.
As explained by my colleague, the field name should match the column name. In your excel sheet, the column name is shown as the field options.
The widget can only be used to auto select any of the options in the form - i.e. Cost Low, Cost medium, and Cost high.
