For whatever reason, our JotForm account (free) has been suspended and we can no longer access our ADMIN account or our online form at https://www.jotform.com/uteplibertz/11304718952.
The error message indicates that the "Form Not Found. This Form is Disabled."
For the record, our JotForm does NOT ask for any personal end-user info such as SSNs, etc., so I am not sure why our account was suspended. We have been using this same form for several years without any problems up until now.
Would you please investigate and advise if access can be restored ASAP?
