Membership Card form

Can I create a form that looks like a Membership Card and can be filled in and made into a printable PDF? See attached card

I would recommend editing that image (using a photo editing application) to remove the names and ID number, save it as a PDF file, then import it to Jotform. Once done, add Name fields to the form and map it to the imported PDF.

Enable sending of the imported PDF to the users (who will fill out the form) by attaching it to the autoresponder email:

So when the user submits the form, they should receive a copy of the ID with their details in a PDF format.
