I did check your account and found that it is currently suspended as your form is suspected to be used for phishing. I am escalating this thread to our form review team to check what caused this suspension.

Hi Mya,
To receive payments, you have to use mastering payment Integration with JotForm: https://www.jotform.com/help/323-mastering-payment-form-integrations-with-jotform
Apart from this, asking for CC details or requiring sensitive information for payment collection is against our terms and contitions and your account will be suspended.
