Not able to login to my account.

    Good Morning,

    I have been trying to login to my account to reschedule a COVID test. My login information will not work and the password reset email does not send to me.

    What is your Jotform account username or the email address you have used in your account profile? I am unable to find any account with your email address and that is the reason why you are not receiving password reset instructions email.

    Alternatively you can also let us know any of your form id, form URL or web page URL where you have embedded your form, and we will take a look. We will wait for your response.
