    Hello,

I'm designing a form for attendees to fill out that are going on an incentive trip. For those that wish not to go, is there a way we can have a multiple choice question at the very top that reads something along the lines of "yes, i accept", and "no, i decline" - then anyone that selects yes will be asked to fill out personal travel info, while anyone that selects no will be directed to a submit button?

    Hi,

you can achieve what you are looking for by using conditionals logics in order to SHOW/HIDE certain fields or groups of fields depending on a previous selection made by the user, could you please check this example I made for you:

    http://www.jotformpro.com/form/32736673242961

Feel free to clone the form to your own use

in this case I use conditionals logics combined with this guide about How to Show/Hide multiple fields based on one condition

    Hope it helps

please let us know if you need further assistance

    Thanks
