    Can we create a logic question

    I'm designing a form for attendees to fill out that are going on an incentive trip. For those that wish not to go, is there a way we can have a multiple choice question at the very top that reads something along the lines of "yes, i accept", and "no, i decline" - then anyone that selects yes will be asked to fill out personal travel info, while anyone that selects no will be directed to a submit button?

    you can achieve what you are looking for by using conditionals logics in order to SHOW/HIDE certain fields or groups of fields depending on a previous selection made by the user, could you please check this example I made for you:
