How can I give users the option to add another field?

I know about the configurable list, but I want it to include more options to add attachments if needed and also have it look the way I want it to look. I provided a screenshot of what I want to be duplicated if the users needs it. Kind of like how job applications have an "add more experience" field that duplicates.

The regular fields cannot be dynamically generated, so the solution is to add the extra fields to the form and show them based on conditional rules.
