Form not accepting submissions

My form is enabled, but is not allowing entries. Please assist.

After our subscription renewed yesterday, our original link
https://form.jotform.com/93084045347155 was changed to
https://form.jotform.com/203205598316152 *We had been using the original
link perfectly since Monday*. Now, we had to quickly notify all of our
customers to use the new link. I don't know why it changed, but would
appreciate it if this could be investigated so it isn't an issue in the
future.

As per checking, the two forms are different:

1. https://form.jotform.com/93084045347155 - W20 Class Submissions for District Funding 
2. https://form.jotform.com/203205598316152 - W21 Class Submissions for District Funding

However, the form https://form.jotform.com/93084045347155 is currently disabled.
