link PDF in the email when the form is submitted

Need help building a link to be used in an external website. I need to create a form that asks for name and email and after that info is captured, the person can download the document link they clicked on. Not sure how the document link fits in with the name and email link. I included an image of what we are trying to accomplish.

It is possible to attach a PDF document in the autoresponder email that your users may download.
