different consent forms based on treatment selected

    Hi,

    is there a way of having different consent forms drop listing based on the treatment selected so just say a client want to have a skin treatment, if they select skin treatments a selection of questions based on that will drop if they select face injection then another selection?

    Hello,

    You would like to know if you can make your form change on the selection the user chose.
    That is entirely possible.

    You will need to use Conditional Logic for your form. Here is the related article: Smart-forms:-conditional-logic-for-online-forms

    To do it you will set up your form with the selection towards the beginning and then all the fields you would like to show for each selection.

    If you add page breaks between each set of questions (e.g. One page of questions for skin treatment, one page for the injection questions and so forth)

    I like to think of the conditions as sentences I would say to somebody to describe the process:

    IF the user selects the option skin treatment, THEN SHOW the page (or the fields) with all the skin treatment questions and HIDE the pages/fields with the other questions.

    That makes it easier for me to visualize what I have to hide or show...

    I have checked your form and it seems you're using the show conditions, however the fields are not hidden. This creates an error with your conditions.

    I would suggest you hide your form fields that you would be using in your condition by default.

    This way, your show condition should work.
