Question about HIPAA Compliance and Website Builder Solutions

I'm working to throw together some online intake forms for a doctor.

If we sign up for JotForm silver, can I design the form in JotForm Builder and then just add it to a Wix or Squarespace website?

And more importantly, will the result be considered HIPAA compliant? Or would I need to use a different host/builder for the form to keep compliance?

Finally would jotform allow me to send a link to patients to fill out the form in HIPAA compliance without hosting on a prexisting website?

I'd like us to sign up but I want to know the steps we'd have to take to use your product with HIPAA compliance and we currently don't have something set up.

Yes, there's an option in the form builder to instantly share the form via email, wherein it will be sent from our server.

You will need to subscribe to either SILVER or GOLD plan for HIPAA compliance eligibility. Once subscribed, you have to enable HIPAA from your account date page.

Once activated, your account and forms should be HIPAA compliant now after that, and there will be HIPAA compliant badge that will automatically show on your account and forms.
