Can I download a signed form that looks like the original I created?

After I receive a submitted form, it doesn't look like the original they signed...it's formatted differently without the questions or the information they reviewed....can I download a signed form that looks like the original I created?

You may try creating a PDF document in the PDF editor and attach it to your notification email.

To create a PDF document, in your Form builder select PDF editor

Once created, you can attach it to your notification email.
