Product Listings + Terms form keeps freezing

I've had at least 7 people tell me they can't submit their forms, or that it freezes. This is NOT GOOD! Help!

Secondly, please let us know for the persons who faced the issue: how they filled the form - i.e.

1. using a web browser or using a mobile device?
2. after saving the form completed submissions from the saved session link?

Meanwhile, I cloned your form and tested it. I was not able to replicate the issue as the form submitted fine.
