Form duplicate content: How to delete duplicate content on form?

I made this form https://form.jotform.com/203398046660054 but at the end it again copy the form content .plz fix why its copying the same data

By copy of the content, did you mean the content below the submit button in your form?
If you want to delete those elements, in the Form Builder, click on the element, and click on the red trashcan icon to delete the items.

Yes content below submit button is repeating .
Form copy page 1 content to below submit button.

You can edit your form in the form builder and remove the other fill in the blank widget below the submit button.
