Airtable fields are not mapping

I have authenticated Airtable, selected the base and table. But the fields are not mapping. I only have the same 3 fields available no matter which I select or even if I switch to a different table. See screenshot.

You are unable to map for fields with Airtable because your form does not have any form fields.

You need to add the fields to your form, to be able to map them with Airtable. For example, if you want to map the Airtables's Full Name field, you need to add the Full Name field to the form.
