    Hi Jotform support,

    I don't know what's going on, but your system has tagged me as a spammer or phishing. I'm not doing anything like that. I've emailed multiple times to get our account back. We use jotform to fill software orders and setup in our business and this has caused a great inconvenience. Please restore my account access.

    Your account was suspended due to collection of multiple login credentials on these forms:

    Logistics: Black Friday Concierge Setup

    Logistics: Concierge Viral Bot

    Logistics: Concierge Birthday Bot

    Logistics: Campaign setup


    This is not allowed as stated in our Terms of Service.


    You were notified by my colleagues 4 times since 2018 that collection of user/login credentials is not allowed here at JotForm. I've also responded to your whitelist request last December 1st about this.


    Despite the notifications we made, you still created a form recently with the same violations which caused the suspension on your account and flagged your account as a repeat offender.

    In line with this, we can only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.
