How many forms are allowed per subscription?

I know, from reading the pricing, that the forms display X number of forms that can be created with each subscription. But is the number "per month", or total - forever?

In the "free" version, there are 5 forms allowed. Is that per month, per year, or for the entire time I have that version?

It's the total forever, only submissions, form views, and payments are reset every first day of the month.
