You can try this workaround for now.

1. Add a Dropdown or Multiple Choice field in your form where the user can choose to cancel their appointment

2. Add a Long Text for the user to enter their cancellation reason

3. Add Get Form Page URL Widget in your form and apply a condition that if the widget contains "edit" show the above 2 fields you've just added. Guide: How to Show or Hide Fields on Edit URL
