Product list: I need to be able to have two text boxes for each product item in my form.

Good afternoon! I'm trying to recreate a form. I've almost completed it but there are a couple of things that I'm having trouble designing. For instance, I need to be able to have two text boxes for each product item in my form. Is this functionality possible?

I need to be able to have two text boxes for each product item in my form? — May I know what would be the use of these two textboxes? In Jotform's product list field, you can only add one textbox and it's used for quantity field.
