Op zoek naar een bepaald offerte formulier

Hoi,

Ik ben op zoek naar een offerteformulier die de onderstaande functionaliteiten heeft, maar ik weet niet welke ik dan moet selecteren. Kunnen jullie mij dit vertellen? Alvast bedankt.

1. per pagina kan ik meerdere vragen stellen
2. bovenaan het offerteformulier staan alle stappen die de bezoeker moet doorlopen om het offerteformulier af te ronden
3. onderaan een cta
4. Het formulier moet er ongeveer als volgt uitzien:
