I've been able to replicate the issue on both of the forms on your account I teseted creating a third form to test the email settings and they do work on the test form. Not too sure why the forms have gotten like this, I also tried clearing you account's form cache but it did not solve the problem. I would suggest on recreating both forms from scratch on new forms, then test again to create the email notifications.
