Hi again,
I see on my cloned form that you have used the Square payment integration: could you please connect and then reconnect the Square integration and let us know if the issue persists?
You can do so by opening your Payment widget to the Square connection wizard, change the Mode dropdown to Live Mode:
and then click the Connect button.
